Question:
has anyone been approved through php of mid michigan

i was just wondering if anyone out there has dealt with php of mid michigan,i am awaiting an approval from them and they have only just recently started including the wls with their insurance,so i am wondering what my chances are with them.please post or e-mail me with any answers    — cheri S. On Thursday my pcp faxed a letter to PHP and by today (Saturday) I had a letter from them saying I met initial criteria, but need to provide documentation of 12 weeks of doctor supervised diets or 2 receipts from weight loss programs, or I could do the optifast program at Sparrow (but that would cost about $75 per week out of pocket). The initial criteria I was told are over age 18 and a BMI over 40. Hope that helps, and good luck!
