Question:
I received a letter from insurance and it says I must reapply if
surgery is not within 30 days. Has anyone else received this type of letter? Does this just mean that I have to get regular pre-approval right before the surgery? — Leslie M. (posted on October 22, 2001)

Hi there, they all say that, but your doctor's office just needs to call in
and talk to them. If they (dr's) aren't able to schedule your surgery
within 30 days, they just need to let the insurance know when it will be.
Usuall you actually have 90 days, but they want a date by 30! Hope that
helps.

It just means you have a 30 day approval for the surgery. If,
for some reason, your surgeon reschedules the surgery his office
must call the insurer and notify them of the date change. If they
fail to do this and the surgery falls outside the 30 day period,
the docs and the hospital won't be paid. They'll have to appeal and
explain why they didn't notify the insurer about the date change.
This is quite routine....your surgeon's office is more than familiar
with all these insurance rules....nothing to worry about.
