Even if it is...lucky you...I'm self pay and still saving..only have
$22,000.00 saved up so far..count your blessings

My co-pay is 10%, and I had to pay up front. My co-pay was $625. I
charged it on my credit card. Plus $164 for my initial visit. My surgery
is in 5 weeks.

whether or not you are going to have to pay your 20% coinsurace up front is
up to the doctor(hospital), usually they will bill the insurance first and
the insurance will pay 80% of the reasonable and customary charge(that is
an amount that all insurers go by, it goes by geographical area) so if the
surgeon bills 40,000. and your insurer finds the reasonable and customary
to be 28,000 they are going to pay the 80% of that amount. If your dr. is
in network then he will accept that amount and you will pay the other 20%,
usually by billing you for it. If the doctor is out of network then you pay
the 20% and that doctor can bill you for the balance as well.
