I have GREAT WEST PPO-a great ins but PLEASE start a file on your weight lost history they want 3yrs which to be truthful; i gave then what they wanted if you know what i mean? It's diddicult to remember all the fad diets and the lil church that held WW meetings which no longer does it??? Also get a dietician consult (i can give you a name of a person who does it over the phone) and the biggest is the psychological consult. This is three things to get your approval. If you have it all to start then you'll hear back with in a few days. Now this is with a PPO which means i didn't need a PCP referal to the surgon-just went on my own. If your non PPO then you'll need to see your PCP first. GOOD LUCK!

Hi there, I was denied by Great West life back in 1994 when I first looke into WLS and they flat out denied me and I was 600 or>. I know INS has come along way so you may be OK... I worked for Domino's Pizza then do you work for DPI?
