Question:
Diet History troubles

I'm trying to get all of my paperwork together for my visit with the surgeon and I'm having some difficulty putting together a diet history. I'm not sure of the format etc. Is there anyone here that has a documented diet history that wouldn't mind sharing? I would only use it as a template.
