DOES ANYONE HAVE A DIET HISTORY

Hi! I am in the process of gathering all my necessary information for insurance approval. The only thing I do not have is a documented diet history. I documented everything I could remember from 1999. Does anyone have a documented diet history you are willing to share with me. If so, please email it to me at [email protected]. hi i had medical mutual and they requested the same thing so i had to send them name of the comapny like weight watches with the dates and so on hope this was of some help.
