Question:
UHC EPO....exclusion question and can it be appealed this way?
the UHC EPO exclusion "no coverage for obese, including morbid obesity" is placed by your employer and not neccessarily UHC. is this correct? if this is true, i am currently working to appeal. but my question is, would it make a difference if i had my employer see that i need this medically and they themselves agree to cover and have them inform UHC to cover it for me considering my specific situaion. — manalden55 (posted on April 11, 2003)

It all depends on which "package" your employer bought, if they
bought a standard package, if they bought a package where they could pick
and choose, if they are self-funded, etc. You just can't automatically
assume that it is an employer's choice. A large company, with many
enrollees would most likely have a plan where they pick and choose the
options they want in the plan, as would a self-funded plan. A small
company is more likely to have one of the "buy-off-the-shelf"
packages that an insurance company might offer, with no ability to change
the options.
