Question:
I need a sample letter for my PCP to write of Medical Necessity

I have a new PCP here in the state of FL.I have only seen him once. I need to get my Dr.'s in Tenn. to write me a letter of Medical Necessity and they both said if I could fax them a rough draft of what I need they would re-write it and fax it back to me. So I am begging for your help. If anyone out there could help me with a rough draft I would greatly apperciate it. Thank you so much.    — Angel S. (posted on March 18, 2003)

My PCP had to write a letter of medical necessity. His was very brief. I also had to write a letter about why I wanted the surgery. Both of these were sent to the surgeon who then added their letter to the package and sent it to the insurance company. I borrowed mine from Debby C. She is in the profile list but email me and I will send you her email. It was great. I just changed the parts that I need for me.

Take a look at my profile, it has a example of the things on my referral to my insurance company, my insurance did not require a letter, they just wanted a referral from my PCP, so check with your insurance first and make sure what they want a referral or a actually letter. Hope this helps
