Is it possible to have a boob job and tummy tuck covered if...

I have a hernia and will need to get it repaired fairly soon. Needless to say, after losing 100+ pounds, I will also need a tummy tuck and boob job. Are there surgeons that will perform all three at the same time, and under what conditions will insurance cover the boob job and tummy tuck? I do get an itchy rash under my tummy and my boobs.
