What do they mean when they say they have a Written Exlcusion Policy?

A written exclusion is a written agreement in a particular policy to
exclude a particular treatment. My insurance had a written exclusion for
gastric bypass surgery even for medical necessity. It was so airtight that
the wording included "exclusion of gastric bypass for the treatment of
obesity and morbid obesity." Obesity and Morbid Obesity are
classified differently and have different diagnoses codes. Some insurance
policies have exclusions for gastric bypass UNLESS deemed medically
necessary. In that instance, you would have to prove to the underwriters of
the insurance company through medical records, letters from doctors etc
that the surgery is medically necessary for you. I hope this helps.
