Need a STRONG letter to submit to insurance company. Appt TOMORROW!!!

HELP! I have my first appointment tomorrow. They told me to go ahead and come in with a letter to the insurance company to help with approval. I have written one, but I don't feel it's strong enough. Everytime I try to rewrite it I get all emotional. If some one could help me out with a strong sample letter or two that would be awesome!!!
