Aetna - Do you know the Panniculectomy & Mastopexy Ins. Code & requirements for AetnaUSHC?

Aetna USHC has their Coverage Policy Bulletins on their website. CPB #211 deals with Abdominoplasty and CPB #1l7 deals with Breast Reduction. GO to the site at:http://www.aetnaushc.com/cpb/cpb_alpha.htm Several codes for plastic surgeries appear in the Third Pary Payer Coverage bulletin for treatment of skin redundancy following massive weight loss. The site for that is: http://www.plasticsurgery.org/profinfo/postpap/skin.htm The code that I see there for pann is CPT 15831 and for mastopexy is CPT 19316. The ICD-9 indications are also listed there...these are the diagnosis codes that the PS uses to justify the procedure.
