Question:
BTS wants medical records if possible when I go to my first appt.
Do I have to bring everything? What can I get by with in the mean time till I know what they want. Is there anything else I can be prepared to bring? Thank you — [Anonymous] (posted on March 16, 2001)

I don't really know the answer. One thing I did was to sort through my
records and list the date seen and the weight recorded so that I would have
a good list of my weight history. It also helped me remember what diets I
was on and when. Your records should also help you recall any
co-morbidities you have suffered from.

My surgeon wanted my medical records for the past 5 years. I just went by
the docs' offices and hospitals and signed a form requesting they send
copies to my surgeon's office. I didn't physically take them there myself.

In response to Cindy's post, I've found that it's better to have them send
YOU the records so that you can keep a copy and take them to your surgeon.
Past doctors have lost my records or have closed their office, so it's
helped me to keep a binder with all the records I can get my hands on.

I just called my doctor's office and told them I wanted to hand-deliver a
copy of my medical records to my surgeon's office. (they asked for 24 hours
to get them all copied for me). Once there, I signed a release form, took
the copies, made an additional set for my files, and took the copies to my
surgeon!
