State of AZ, Maricopa County and City of Phoenix all have insurance programs that pay with proof of medical necessity. I was approved on first appeal and have CIGNA HMO. good luck to you!

First Health covers it if it's medically necessary. There is a $1500 deductible and if you use someon in thier bariatic network then they pay 90%. At least that's what it said when I looked it up. I work for First Health and checked my employee web site.
