Where do I go now?

Hi, I'm a 28 yr old single mother of 2. I am 5'4" and weigh 300lbs. I live in the western part of New York State and work a job that offers no insurance due to the temporary status of my position. I make $20,000/yr and cannot afford to self pay. I make too much to qualify for medicaid. Thank the lord that my children are covered by NY's 'child health plus' plan. I don't know where to turn now. My credit is not good at all, so loans are out of the question. I am desperate to have surgery done. Emotionally I am not doing well. Being overweight is upsetting and soooo depressing.
