Hi. I'm not sure but I think it has to say somewhere that the surgery is
medically necessary.

Here's my advice.... EXPAND, EXPAND, EXPAND! For example, her GERD is being
treated with Nexium which only gives sub-optimal relief and is not a
solution. Her GERD causes her to have pain each day in her chest, which
leads her to *throw up, get sleepy, whatever*. The *vomiting* whatever has
been detrimintal to her mental health.... Are you getting what I mean???
Tell about how it effects/affects your life. Make it personal. Complain
about every single pain you have. Unfortunately insurers in the US are a
pain and the almighty dollar is what they care about. You have to realize
they don't care if you die, it would save them money. Tell them why it is
cost effective for them to pay for this surgery. Good luck to you.
