Does anyone know what the requirements for qualify for WLS with TRC?

This list of criteria for WLS thru TRC comes straight from the requirements they gave me 2 months ago. 1.PCP must make referral for surgical eval. and intervention to a surgeon who is experienced and active in surgery procedures for the morbid obesity. 2. Must be evidence of attempts at medicaaly supervised weight loss. Documentation must be provided by supervising Doc. and must confirm 12 months of participation in weight loss program that includes at least monthly visits. 3.Using the Metropolitan Life tables of height and weight, you must be double normal weight. 4. health must be stable, no uncontrolled diabetes or hypertention, no congestive heart failure,no renal failure, no COPD. 5. Psychiatrist or Psychologist must state that you will be a good candidate for surgery and post-surgery compliance. The TRC branch that I am going thru gave this list to me, without hesitation. I see them 7/15/03, about my chances for approval wish me luck!
