The insurance company is requesting a nutritional evaluation.

When my insurance company requested a nutritional evaluation, I called my
doctors office! My doctor has a nutritionist that he works with and she
called me back the next day. It was actually pretty fast because she
reviewed my chart and then gave me a phone interview. The nutritionist
then wrote up the evaluation and it was faxed to my insurance company
within a week. Hope this helps!

My surgeon requested the same, and it wasn't for insurance purposes, it was
for him to finalize his decision to do surgery. Mine consisted of a meeting
with the nutritionist and a complete blood nutritional evaluation. They
took tubes and tubes of blood... even covered one with foil and wouldn't
let the light hit it because it would ruin the test. So... it could be just
a "paper" evaluation, or they may include bloodwork. Either way,
it isn't necessarily something that the insurance can use to stop you from
having surgery. It is actually a good thing. It is the best way to keep
track of your nutritional health and continued wellness after surgery.
