Question:
Has anybody recieved approval from an ins. co when they had a written exclusion?

YES, but my employer added a rider to include the coverage. It added a bit to the premium, but they dropped maternity for now and it came out the same. I showed them the legislation about covering obesity and morbid obesity and they were willing to help. And other pts in my md's office said that they had to only pay the surgeon adn the hospital was covered. GOOD LUCK
