how rar back does suprvised weight loss have to b w/ bcbs/fl/blu choice?

I know the girl from bcbs customer service just told I needed supr.weight progran for 12 months but how far back can that b from I dont have recent ndocumented only dr. supr. fron 1996,Can someone tell me who had expierience w/ bcbs/ppo/fl.Thank you all very much    — Kelliann S. Kelliann- This is a fairly new policy with BCBS (one year sup. weight lose), so you may need to call them and find out the answer.
