when insurance want 12 mths. supervised weight loss history how far back

my ins. (aetna) required a diet history with a 2 year supervised diet. they did not specify a time frame it had to have been in. I did not have 2 years, i had less than 1, but my surgeon submitted the request anyway along with my co-mobiditys and i was approved in 48 hours.

Mine covered a span of about 10 years and got approved.
