when the ins. wants 12 mths. supervised weight loss how far do they want it from?

how far back do they want it from? I only have mine from 1995-1996 after that it was on my own or weight wathchers jenny craig etc...Could it be from that long ago when I called my ins. comp. bcbsfl they just told me I needed 12 mths.supervised weight loss,Could someone let me know there expierience on it.THANX in ADVANCE    — Kelliann S.
