Question:
Does anyone have United Health Care-EPO?

I also have UHC EPO and on the website, (myuhc.com), it says that they will not cover plastic surgery if it is cosmetic. So I would say be prepared to prove that it is not solely cosmetic. Be prepared to prove that it is medically necessary.
