I have Med.Mutual of Oh. The Bariatric Tretment Center said they could get it approved last August. I recieved a letter requesting further documentation on five years worth of weight and health records from my regular doctor. Finally, all was approved & I'm set to go in the end of Jan. It my take a while to get approved, but hang in there & good luck
