Has anyone had surgery with Dr. Vafa Shayani of Illinois????

I had not had surgery with him, but I did have a consultation, I found him to be very personable, and reasonable. He has a great staff, however, I am actually waiting for a letter of refferral from his office, which I have been waiting for for almost three weeks now.
