Not exactly sure of the question. But if you are asking about being
approved for wls. I have Anthen BC/BS Blue Preferred Primary Plus(POS)of
Ohio. I was approved after 1st attempt. Within 12 days. No medically
supervised diet history. Although my PCP did send a letter to my surgeon
addressed to whom it may concern .. stating that I had attempted many diets
while under his care. But he didnt supervise any of them. I think my
success came from following the advice of the surgeons office personnel in
being patient and getting all the important info together before submitting
anything & the support of my PCP. My submission included 5yr previous
medical records. Diet history. Sleep study. Psych evaluation. And letter
from my PCP stating his belief that I was a good candidate for the surgery.
I contacted the surgeons office in Feb. Initial consultation with surgeon
4.21. Psych evaluation 4.13. Initial consultation with sleep Dr 4.13.
Sleep study 5.18-19. Predetermination submitted to insurance 6.11.
Approved 6.23. Waiting on surgeon to set surgery date. Just a little
timeline. Not sure if this helped. Good luck to you. PS ... I called the
insurance everyday after the predetermination was submitted. I was
extremely courteous to the customer service employee with which I spoke.
She was always professional & checked the status for me. When still in
review she encouraged me to call back the next day and check again.
