PROOVE IT !!! HAS YOUR INSURANCE EVER ASKED YOU TO ??
Did your insurance company ask for written proof or verification of the "Weight Loss Attempts" you listed in your DIET HISTORY. (Not the doctor supervised ones) For eg, if you listed Jenny Craig for x-months with a weight loss of x-pounds, will they ask for written verification of the dates and regular weighings from the CENTER??? Nope they never asked, but I made sure I only put down the things I had
really tried in case I was ever questioned. That way I knew about each
program, how they worked, etc. Not to imply that was your meaning :>)

This is not part of the criteria my insurance company uses, so, no, I never
had to prove it. My surgeon did have a huge list that I had to check off
on all the different things I tried.

The only thing that my ins co wanted was written proof of the doctor
superivsed weight loss attempts, so all I had to do was get my medical
records and submit them to the ins co. They werent interested in my other
weight loss attempts. Hope this helps!
