Question:
I need help condensing elaborate diet history into what Dr or Insurance needs/wants
Any help on how to condense diet history into what Dr/Insurance wants/needs would be greatly appreciated. I have 15 lines to complete and as any of you can imagine probably at least 150 diets in my history. There is no way I can possibly remember such details of those diets. Sarah, now did the doctor specifically tell you to condense it? Or is there
just 15 lines in the spaces provided in the information papers? If it's the
latter, just type up your own sheet and make it however long needed. Mine
came to two pages with diets, gym memberships, videos, etc. If it is the
first, I know personally I've been on a billion diets as well, but many of
them have been repeated. The same for you? Then write Weight Watchers:
years 1990, 1995 and 2001, etc. And maybe you could pick the top 15 that
are the most recent? Hope you find what works!
