Generally speaking, only address the reasons given why you were denied. Do
not include a lot of emotional stuff. I can tell you from experience, that
turns the reviewer off; it does not help, just makes you look like a
whiner. Any time you can justify surgery based on future cost savings to
the plan, that's where you need to go. If you give more detail on why you
were denied, that might generate more responses.
