Question:
Can I get feedback on this type of records and if its enough documentation?
I just obtained my medical records from 1995 to the present ... that is when I started seeing my doctor regularly--for almost every visit I expressed concerns about my weigh. My dr always notated the conversations including what we discussed, ie diet, exercise, etc...will this count as 'dr supervised weight loss attempts?' My weight is noted in each record alongwith all medications for all my ailments! Specific diets are not mentioned but she mentions over and over again that I am trying but not having any success, including Xenical. I have Aetna Open PPO and will be seeing Dr. Patterson in Portland Oregon if that helps. — jenn2002 (posted on October 1, 2001)

I had a similar problem. I made a "Table" of each month for 18
mos. weight, how much I gained or lost, what sort of diet I was on. Med
changes. Diagnosis Changes. MD I saw and a little note about how I was
feeling about it, depressed etc. I have B/CB/S, it worked for me. Good

That was all I needed for my coverage - just notes in the doctor's chart
regarding my weight loss efforts. My insurance was HealthNet HMO. -Kate-
