Help! Need sample of letter of necessity from PCP

I need a letter of necessity from my PCP before I can be approved for surgery. I was hoping to get a good sample to show her to speed the process along.    — michelle C. (posted on August 11, 2004)

I wish you the best of luck on your journey....my ins has a waiting period and I just found this out yesterday ......I can't have my surgery until after Jan 05......this was heartbreaking news...........but I am hanging on here so I can be of support to everyone else !!
