Go to the library and search for insurance letter samples, insurance letter,
or sample letter. You should find lots of help there.

Go to http://www.obesityhelp.com/morbidobesity/m-ins-lettermn.phtml I
used this format to write up my letter. I have it on my profile if you want
to take a look at how it turned out. Good luck, Robin

Be as detailed as possible, in fact, be overly detailed. Don't skimp. If
you feel sick of writing set it down and come back to it a day later. You
will save yourself a ton of grief if your letter contains every detail
about your family history, your history and your medical condition. Put
down every diet you've tried, when and how much you lost. Be very
detailed.

I have not been approved yet, but I have written my letter. The number one
piece of advice I can give you is DO NOT even HINT at you wanting to have
the surgery for any cosmetic reason. Follow everything up with a medical
reason. Also, send in pictures. I have heard it is very helpful if they
can put a face to your case. I don't know if you have children or not, but
I also told how my weight kept me from being able to interact with my
little boy. Hope this helps!!!

I wrote a letter to my company. They did not require it but I thought that
if I could let them get to know me a little then maybe I just wouldn't be
just a bunch of papers to them. I explained all the diets I had been on and
how much I would love to be able to be an active part of my childs life. I
also let them know how much thought this process takes and that it is not
just a instant thing. I explained that I knew it would require a big change
and that I had dieted my whole life and that I was ready for one to work
long term. My letter was only a page and a half as I know they are busy and
don't have time to ready all my complaints. I poured my heart into that
letter to try and get them to "feel" how hard it is to
overweight. I was approved in 5 days.
