Question:
Has anyone gotton approved through Great West?
My current carrier is Great West. I have their POS plan. They will not give me any information until after my Bariatric Doctor appointment. Was wondering if anyone else has this carrier and has been approved or denied. — Teresa B. (posted on July 18, 2003)

I have Great West (although its a PPO not POS) and I was aproved on the 1st
shot. It took a while for them to reach a decision though. You can read the

I also have GWL as a PPO and was approved on the first try. Their
requirements, for my plan at least, were in line with what my surgeon had
me doing...psych eval, some diet history, at least 25 yro, bmi 40, list of
co-morbs.
My Dr's office took care of all this.

I have Great West PPO and was also approved on the first try. Required
diet history, 100 lbs or more overweight, psyc evaluation, letter of
medical necessity. Nothing out of the ordinary.

I was also approved on the first try. I had no trouble getting the list of
requirement, but once my doc officially requested auth, the actual list of
criteria was slightly different. You can email me and I can send you the
list if you want, but basically call the Utilization Review department and
they will give you the info. Supervised diet for 12 months, no psych
diagnosis, no drug/alcohol addiction for xx months, postop regimen/support
was a biggie.

I also have Great-West PPO. Had no trouble getting approved. Usual
requirements only.

I have Great West PPO and I was approved on my first try. It did take 12
weeks for approval, and I had to have a psych eval., TSH (lab
work),detailed diet history, BMI >40 and had to be 100 lbs overweight.
They were nice and they have paid all but 10%. Kelly Lap RNY 4-21-03
