Question:
Does anyone know the criteria for wls thro bcbs Blue Choice?
I would like to know if i meet the requirements for wls. My insurance co is BCBS of Michigan Blue Choice. I can get the info from them with a procedure code but I wanted to check it out befor my Oct.17th 1st consult with the surgeon. I am not sure if this will help, I have bcbs of ILL. I was approved almost
immediatly, I am 32, have not had any big comobities,just a BMI of 48, I
don't know what blue choice is, I have an HMO.Hope this helps alittle.

Hi, I have bcbs blue choice ppo ,but in fl. I called the insurance comp.
finally for myself yesterday because b 4 that I was afraid to call I dont
know y But anyway she told me I needed a pysch.evaluation,obesity related
history that was medically supervised for 12 months(i.e.
diet,exercise,medications,etc.)That is all she told me But trough this site
people also told me I would need thyroid testing.And in my benefits booklet
it says u have to have a bmi of 40 or be 100 pounds overweight. But people
also told me you need a letter from your pcp stating that they support the

I too have BCBS Blue Choice in IL and was approved immediately. The only
pre op testing that was required was a breathing test, gall bladder ultra
sound, and blood work. I wasnt even required to diet or have a physc exam!
I had no health problems whatsoever, just a BMI of 42. They seem pretty
easy on the approval. I was SURE I would never get approved, but to my
suprise within 2 weeks I recieved an approval letter and had surgery 5
weeks after that! So never give up hope! Good luck to you!! Tonya
