I had this trouble too. It took me 4 months to get a psychologist who
would see me about this. My insurance would tell me that they had no
psychologists who were "in network" who would see me about
Bariatric Surgery, yet they did give me names to call. I called and called
and called some more and finally after about the 3rd month I figured out
what to ask for. Call a psychologist on your insurance and ask for an MMPI
exam. Usually this is just a one session deal and once they realize this
is all you need they will more than likely see you. I don't know of any
psychologists in your area but if you call around and ask who does MMPI
exams your bound to find someone or someone who knows who does. I even
called the BTC in TX to get names of psychologists they recommended. I was
willing to drive the 3 hours to get this done!! I hope this helped some.
Take care and good luck! ; )

I also had my consultation at BTC in Ypsilanti, and am currently trying to
get the surgery approved through my insurance company. I had my
Psychological exam with the following psychologist:
Jan Hansen, Ph.D
101 S. Huron Street
Suite 203A
Saline, Michigan 48176
She is my regular psychologist (now) but she did the psychological
evaluation for me on my second visit. She has done several evaulations for
BTC so she is familiar with the process.
