Your appeal letter is going to have to be based upon your medical need for WLS. But there is not much in your profile to go by. Do you have serious weight related comorbidities? Sleep apnea, High Blood Pressure, diabetes, etc. Focus on the health problems you have and how the best solution and cheapest in the long run for the ins. comp. is for you to have WLS. What costs more a gastric bypass now or a coronary bypass later? Insurance companies are really more concerned about money than they are your health, so show them how they can save money in the long run by approving your WLS.
