I need to write my first appeal letter , any-one have any advise??

I got copies of all my medical records and attached them as back-up.
Highlighting any notes that I felt would support my case, that way they did
not need to go and follow up with all the doctors. Check out my profile

Everything depends on the reason for the denial. There's no
point in sending medical records if the denial is contractual.

I would get your insurance companies "requirements" in writing..
Then gather all of your information, crossed reference to their
requirements, write your letter, your doctor's request of pre-approval, any
other specialist you have EVER seen for ANYTHING obesity related (heart,
lungs, other organs, joints - knees ankles hips, OBGYN - abnormal paps,
cysts, "high risk" pregnancy ect.) should write a letter and
include the problem and treatment, they may request "medically
supervised diets" don't sell yourself short, make a list of the
un-supervised diets too, all they can do is trash it, but remember these
are people too reading these.. Include a full body picture (not
nessicarily unclothed) that shows exactly "how big" you really
are (no offense, but we all hate those pictures I know I hated mine) Since
apparently this is something they do cover (I was reading your struggle
with the first go round) make them pay for it! If you would like a copy of
some of the letters I've collected and used in my process please feel free
to email me.(I fought my insurance for 9 months, denied twice, not the same
