I got a letter similar to that.
It was a 50% co-pay because the doctor and the hospital is considered out
of area.
I called my insurance and explained that I had no choice but to go out of
area because there is no doctor that does this surgery where I live. The
insurance company then told me to go to the insurance website and print out
all of the approved providers in a 20 mile radius of my zip code. I did
this, called all the doctors on the list and noted next to the name their
answer, NO. The i faxed this to the insurance and they overturned the out
of network to in network. Hope this helps. Good luck
