In my opinion, through reading many insurance comments on this and other
sites, it seems that United Healthcare(HMO and PPO) approves quickly and
overrides its own exclusion on a regular basis... especially when you go to
a major center who does many of these procedures.

Rebecca, I originally had United Health Care, about 2 yrs ago I guess. I
had started researching surgery and was well on my way, and decided to try
once again on my own. I switched over to Kaiser. I obviously didn't succeed
on my own and I started heavy research again. I had heard that with United
Health Care, as long as it was medically "necessary" they would
approve easily. So I switched back and true to their word, I have not had
one problem at all. Surgery is set for April 28th. My consultation was
March 6th. It only took about 2 days for them to approve once they had all
of my information from the doctor. I have UHC select choice POS. I did't
have to go through a primary care physician, I got to go straight into
consultation with a $15 co pay and that was it. If this is a choice for you
I would highly recommend it. Good Luck!
