Question:
does anyone know if united healthcare PPO require the 6 months of weight loss attempt records ?

It all depends on your insurance company. I had to list the diets I had
been on previously, but not six months straight, nor did they ask for proof
of it. Good luck!

I have uhc pos but as far as I can tell, UHC usually doesnt require
anything like that. They told me they require your bmi over 40 and at
least 1 comorbity. good luck...jeannie

I have United Health plan for out patient and BC/BS for hospital coverage
here in nyc and they asked for nothing. claim was submitted 11/19/04 iwas
approved 11/22/04. Dr. love this plan it gives them no problem and they get
paid quickly. I love them

I have UHC PPO . I am from Ia and I had to have proof.Not only that but
they reqired proof of several other things. for ex. I had to get my Dr.
records and prove I was obese for at least 3 years in a row and that I had
sought help. (Although they do not pay for the help) But maybe that is
just how my policy was written.

I have UHC/POS and I did have to show proof of being obese for the past 5
years and a 6-month doctor supervised (or licensed nutritionists) diet
within the past 12 months. I think it all depends on who administers your
particular plan. The good news is once I had everything UHC wanted, I had
an approval in 2 days. :)

I know folks who have had WLS and had BC/BS. They only had to provide
minimal info before they got approval! I have Kaiser and had to provide my
weight loss efforts over the past 5 years plus dr and nutritionist
controled diets I had been on. Anyway good luck!
