Question:
Does anyone have a copy of their own letter 2 insurance asking 4 approval?

I am trying to write my letter to my insurance company asking for approval for my surgery and know I can't make an emotional appeal because these people aren't "emotional" people. They are based on facts and figures. Does anyone have a copy of their own letter they would be willing to share with me so I can get a better idea of what I am supposed to be writing? The one the website offers is good, but the co-morbidities it talks about don't pertain to me and I am scared to death I am going to say the wrong things. LOL... look at me... Go to the stages of your journey section at this site. The letter has been written for you. Add your own co-morbs, bmi, weight etc...change what you need to change...pretty much everything that you need is here! Just make sure to include a list of all your dieting attempts, medications, wl meds, clinically supervised diets and the lot. Good luck!
