Ask for Ginny. She works for Dr. Kane.

I have Humana Premier HMO, my understanding is that Humana has an agreement
in place (fixed pricing) with Dr. Vitello out of the University of Illinois
(Chicago). That is the ONLY doctor they will pay for. I've gone to him
and barring any problems with Humana, I'll schedule my surgery soon. I
like him a lot!
