Have you looked in the archives? United gets alot of "air time"
on the Q&A bulletin board so there is probably quite a bit in the
archives. I remeber reading that there are different policies for this
insurance and you will probably want someone more specific to yours - PPO,
HMO, State etc.

Hi Mary. I am a UHG employee... sad to say it is an exclusion to our plan
as employee's. I had the surgery, but went on my husbands plan to get it
covered. UHG's policy covered my pre-op appointments and nutritionist
appointments...but nothing associated with the RNY. I do know people who
work here and had it covered by going through several appeals, etc.
However, those individuals had their surgeries a few years ago. It is a
pretty tight exclusion for us as employees. Feel free to email me
individually if you'd like.

I work for uhg employee care coodination, policies and coverage do differ
between us uhg employees, at the time of my surgery,12-01, it was an
exclusion, it was covered under my secondary insurance, the only way you
will know is by contacting benefits, if they say its an exclusion, you can
appeal, they should be able to tell you how, if its not then once you have
had your surgeon's consultation you will need to call care coodination to
start the review process where if you dont meet criteria it still could
become an exclusion. i am going out on a limb b/c i have identified that i
actually work in review, so please respect that this information in this
format is probably even more then I should have posted. Best of luck to you
though.

I worked for UHG / Uniprise when I had my RNY surgery in 10-01. As of
01-02 it became an exlusion for UHC employees. :(
