Hi, I noticed that you are in California. You can go to this website (I
did) and print out the form http://www.edd.ca.gov/diind.htm, or go to your
local EDD office and ask for a form. Good Luck.

I would call a lawyer who specializes in disability. Sometimes the only way
to get it is to get help from one.

Call your local Social Security Admin. Office they will tell you where you
can get the paper work to fill out.

I see someone already gave you the CA EDD website. You could also see if
your employer has them. I work for a fairly large company and they have
this wall outside of the HR office with a TON of forms for various things -
amoung others the state disability form.
