Question:
Does any one have a templete for medical nessesity letter

my doctor has ask my to write up my own letter and he would put it on letter head and add or take out as nessessary. This is what will then will be forwarded to the insurance company. So I want to make sure it sound and looks good.    — lalulan (posted on November 13, 2002)

Hello Alana - I have a copy of the letter that I typed up for my primary doctor in my profile. All she had to do was sign it. Just click on my name at the end of this post, it'll pull up my profile. Once it's loaded, scroll down a little bit and it'll list the different areas of interest in my profile. Click on "My PCP Referral Letter", hope that helps.
