Does any on have a letter from their orthopaedic dr

I see an orthopaedic dr for a hip injury and back pain. I am wanting a panny and a breast reduction. My doctor said to write the letter and he will sign it. Cause he doesn't know what the insurance want. Maybe he is just lazy. Does any one have a leter I can use as a sample on what to write?    — michellemark2764 (posted on September 13, 2002)
