Someone put me in contact with your Vocational Rehab case worker

Can someone give me the telephone number and/or e-mail address of a vocational rehab worker who helped them get surgery from their respective state? I asked this before, but didn't put vocational rehab in the question title, and was afraid the appropriate people weren't clicking on my question.
