Yes, I have Anthem and they paid everything except 5,000 total, I believe.
They gave a discounted amount of 16,000. My bill was 46,000 total for
everything and they discounted it to 30,000. Out of that I will be paying
around 5,000. That amount is from different areas so I will be paying
different doctors or services. Good Luck

Hi I had open RNY April 24 '03 and Anthem BC BS paid 100% of all my cost, I
had a bmi of over 50 with several comorbids. You might check with them but
I thought you had to have a bmi of over 40 or at least 35 with comorbids,
ie. high blood pressure, diabetes,GERD,arthritis. Good luck, I didnt have
any problem with them, but they did require more info and I got it to them,
and was approved at 100%.

I also have Anthem BCBS. I do think it probably varies from state to
state. But by the time I had mine in July, I had almost paid my max out of
pocket for the year. So my surgery was covered completely, I didn't even
have my 10% copay by that point. However, my surgeon required an assistant
and the maximum they would pay for this was not enoough. So I did end up
paying for the remainder of that which I think was around $400. I hope
that helps.

Hi--I had Anthem BCBS and live in rural Colorado (it varies I'm sure)--I
had to pay deductible (1500 for me) and "out of pocket" costs of
1000, for a total of about 2500 (actually the bill is approximately 2600).
