Can someone out there please send me some sample appeal letters?
I was denied yesterday. I would like to send in an appeal letter ASAP. I am unsure as to what specific information is needed in an appeal letter. PLEASE HELP!!! — michellelj19 (posted on April 12, 2002)

I don't have an appeal letter for you, but I was wondering what the basis
for their denial was? It might be helpful for those who do have appeal
letters to know that, so you can get specific appeal letters that would
pertain to you!

This site has all three samples of appeal letters and will tell you if the
state you live in has an external appeal you can go after the third
appeal.www.insure.com just type appeal in the search and you will have a
self guide to appeals.

Well a lot of it has to do with what the reasons they denied you. Do you
have serious co-morbidities? Do the exclude WLS all together. I know you
are in a rush to hurry and send in an appeal letter as soon as you can. But
you would be better off taking your time to do it right, rather than
hurried. Do some research, get a letter from your family Dr. Get copies of
medical records, diet history, etc. to back up your request for an
approval.
If all else fails in your own appeal letters, (as was the case with mine),
I would recommend Walter Linstrom (obesitylaw.com). He is a lawyer that
specializes in helping people appeal insurance denials. (He had the surgery
himself several years ago). It cost me $350 for a very detailed analysis
of my policy, my medical history and a 28 page legal brief/appeal letter.
After fighting with my insurance for 4 months, they approved me within 3
days of getting his appeal letter. It was well worth every cent.
