We have a self funded plan, I am looking for literature to take to me employer

I have self funded insurance through my employer. I am trying to arm myself with information to take to my employer when/if the time comes. I work for a company of about 200 people and when I appeal to the company I will be speaking directly to the president and vice president (a brother and sister act). I am looking for literature that I can take to them. Such as future health costs if I do not have surgery, future possible health problems, other self funded companies that have chosen to approve surgery, etc.    — Janine S.
